Care is personalized and designed to evolve with you, offering clear guidance and direction at each stage of the process. You’ll always understand what we’re focusing on, why it matters, and how it supports your overall progress.​

 

Initial Consultation

75 minutes |  $350

During your initial visit, time is spent thoughtfully exploring your health concerns, goals, and health history to create the foundation for a personalized care plan.

This extended visit allows us to slow down, ask thoughtful questions, and explore factors such as symptoms, lifestyle, nutrition, stress, and sleep that may be relevant to how you are feeling. Appropriate lab testing options are then reviewed together, with the most relevant panels selected based on your individual needs, priorities, and resources.

Initial Follow-Up

60 minutes | $325

During your initial follow-up, we focus on helping you understand your results, how they relate to your symptoms, and what patterns may be pointing to deeper imbalances in the body.

This visit is designed to bring clarity to what your body may be communicating and where support is most needed.

Your care plan is thoughtfully built around results, symptoms, and what feels sustainable for your life. Support may include targeted supplementation, nutrition and lifestyle guidance, prescription options when appropriate, or additional referrals when needed.

Ongoing Follow-Up

45 minutes | $225
60 minutes | $300

Functional medicine is a process, not a one-time visit.

Care evolves over time as symptoms are reassessed, labs are reviewed, and your plan is refined based on how the body is responding.

Ongoing visits provide an opportunity to evaluate progress, address new or changing concerns, and adjust your plan as your needs evolve.

Appointment length is based on the complexity of care, current concerns, and the level of support needed at that stage.

Do You Accept Insurance?

At Rooted To Bloom Functional Medicine, we provide personalized, one-on-one care through a cash-pay model and do not contract with or bill health insurance plans. Superbills may be provided for patients seeking possible out-of-network reimbursement from their insurance provider.

We have formally opted out of Medicare and may provide private-pay care to Medicare beneficiaries who complete the required Medicare Private Contract. We do not participate in Minnesota Medical Assistance (MA/Medicaid).

We accept major credit and debit cards, cash, HSA, and FSA payments when permitted by the individual plan.
