2. Set up your patient portal
Your secure patient portal is where you’ll complete your new patient paperwork, review required forms, and upload any relevant medical records or previous lab results.

3. Complete your paperwork
Your intake helps me learn more about your health history, concerns, and goals before we meet. Please complete your required forms before your appointment so we can spend more of our time together focused on you.
